Related Services List for Plan B (Long-term Care)

Related Service Fee
Pharmacy services for Plan B beneficiaries associated with PharmaCare drug benefits. These include:
  • Dispensing
  • Medication review services
  • Special services/professional intervention
Capitation fee of $65.00 per month
Compounding prescriptions The maximum compounding charges that PharmaCare will reimburse are as follows:
  1. CADD injections: $20.00
  2. capsules: $0.30 per capsule
  3. creams/ointments/lotions:
    1. total volume: < or equal to 250 gm/mL: $15.00
    2. total volume: > or equal to 251 gm/mL: $20.00
  4. intrathecal injections: $40.00
  5. oral lozenges: $40.00 per 10 day or greater order*
  6. oral suspensions: $20.00
  7. sterile eye drops, non-preserved: $30.00
  8. sterile IV, IM, SC injections: $20.00
  9. suppositories: $40.00 per 10 day or greater order*
*Compounding charges for suppository and oral lozenge orders for a days supply of less than 10 days must be pro-rated at $4.00 per day.
