Organizations developing interfaces to Ministry of Health (MOH) health information exchange (HIE) systems will submit the following forms (as described in Volume 1 of the conformance standards).
The applicability of each will depend on the organization's type of application and the HIE system being accessed.
Each of these forms, and the MOH’s conformance process, will be discussed and clarified through the organization’s engagement with the MOH’s Conformance, Integration & Standards (CIS) team.

The forms include:
- Request for Integration Services (RIS) – HLTH 4637-8112 2021/04/15 (PDF, 770KB)
- This form is submitted by the software organization acknowledging they have read the conformance standards and are ready to discuss the integration approach with the MOH.
- Vendor Discovery Session (VDS) Questionnaire – HLTH 803 2024/06/04 (docx, 73KB)
- This form is submitted by the software organization to provide further detail in preparation for meeting with the MOH regarding the integration request.
- Vendor Participation Agreement (VPA) – HLTH 4631 2024/04/02 (PDF, 919KB)
- Software organizations must sign a VPA prior to being granted access to MOH environments; and should consult their own legal counsel.
- Conformance Initiation Notice (CIN) – HLTH 4636-8111 2023/02/27 (PDF, 762KB)
- This form is submitted once the organization has completed application development and is ready to proceed with conformance testing and evaluation by the MOH.
- Conformance testing will include both self-testing, and then formal conformance testing.
- All conformance testing will be conducted in a dedicated conformance environment.
- Privacy and Security Conformance Gap Analysis – HLTH 804 2024/10/17 (xlxs, 184kb)
- This is a fillable form which is part of the assessment for conformance to Volume 2 – Privacy and Security.
- Software organizations must complete it to identify if any gaps exist and submit it for Ministry review before they are permitted to connect to the ministry systems.
- PharmaNet API Gateway Self-Testing Checklist – HLTH 4630 2023/02/21 (PDF, 138KB)
- This form is submitted when the organization has completed development related to the PharmaNet API Gateway and is ready to confirm conformance self-testing with the MOH.
- Application Release Assessment (ARA) – HLTH 4635-8110 2021/04/15 (PDF, 807KB)
- Software organizations must complete and submit the ARA prior to implementing changes (e.g., application releases, service upgrades or training) in production.
- Applications for Access to Provincial Health Systems and/or Online Services
- Requests for access to Provincial health systems (e.g., Client Registry, MSP Direct, PharmaNet, Provider and Location Registry) and/or online services (i.e., Provincial Clinical Viewers including CareConnect and Fraser Health’s “UCI”) must be approved by the system owner or administrator and meet all requirements (e.g., signed agreements).
