Medical Assistance in Dying
Last updated on July 5, 2023As of Nov. 1, 2018, the requirements for reporting on medical assistance in dying in British Columbia have changed. All required provincial forms for medical assistance in dying must now be submitted to the B.C. Ministry of Health, which is the designated recipient of reportable information from physicians, nurse practitioners and pharmacists.
How to Report a Medically Assisted (MAiD) Death
The provincial standardized forms for medical assistance in dying are now available on the Forms page on the Ministry of Health’s website. These provincial forms are to be used by patients, practitioners, and pharmacists, and must be downloaded from this website each time they are required, to ensure the most recent version is used.

When to Report a (MAiD) Death to BC Coroners
After Nov.1, 2018, a MAiD death in B.C. will only be required to be reported to the BC Coroners Service if:
- The underlying condition leading to the request for MAiD relates to an accident, violence or self-inflicted injury, or
- The death occurred in an institution referred to in Section 4 of the Coroners Act.
In those situations, the report should be made immediately following the death (or the next business day if outside of normal business hours) by calling 1-855-207-0637 to be connected to the coroner on call.
This reporting requirement is in addition to the obligation to report to the Ministry of Health’s MAiD Oversight Unit.
