Form for applying for direct deposit

Sign up for direct deposit if you are:

  • a child care provider providing licensed, licence-not-required, or registered licence-not-required child care
  • a parent eligible for the Affordable Child Care Benefit for child care provided in the child's home

To set up direct deposit or to update your banking information, submit the Direct Deposit Application (FIN 312) (PDF) through the following methods:

Allow 6 to 8 weeks for processing of your direct deposit application. Payment will be made by cheque until direct deposit is set up.
