Information on medical transportation for services providers supporting income or disability assistance clients

Essential medical transportation support may be available for those who need it.

To qualify for support, your client must either receive: 

• Income or disability assistance and be eligible for general health supplements
• Medical services only

If your non-ministry client has a life-threatening health need (LTHN), they may also be eligible for medical transportation. They must apply for assistance first.

If they’re eligible, the least expensive, appropriate mode of transportation may be available when:

• They require essential medical treatment
• There are extraordinary transportation costs that are significantly more expensive than regular cost of living
• They provide a list of the expected medical transportation costs
• All options and resources have been explored

Required documents

For local medical transportation, provide written verification from a medical practitioner, including:

• Confirmation the treatment is covered by MSP or the Hospital Insurance Act
• The number of appointments per week for treatment
• The expected duration
• Whether or not an escort/attendant is medically required
• The reason they’re unable to use public transportation (if applicable)
• The requested alternate travel method
  • Use a HR3320 form

For non-local medical transportation, provide a completed HR3320 form and either:

• Written verification of need from a medical practitioner
• A Travel Assistance Program (TAP)  form with confirmation number

Payment/invoice process

Once travel or accommodation has been provided, you’ll submit the purchase authorization and original invoice:

• Online with the Service Provider Portal
• By faxing them to our Accounts Payable Department at 1-866-399-9350

When we’ve received the documents, we’ll pay for the travel and/or accommodation by:

• Cheque
• Electronic funds transfer

Payment method depends on the company’s profile in our Corporate Accounting System.
