Approved Standard Diagnostic Outpatient Requisition Forms

The following standard requisition forms have been approved by the Medical Services Commission for use at accredited outpatient diagnostic facilities throughout British Columbia. For additional requisition forms please visit Pathways (log in required).

The Medical Services Commission's Revised Electronic Storage of Diagnostic Facility Requisitions (PDF, 60KB) outlines the requirements for payment of diagnostic services claims associated with requisitions that are stored electronically.

BC Guidelines are clinical practice guidelines and protocols that provide recommendations to B.C. practitioners on delivering high quality, appropriate care to patients with specific clinical conditions or diseases.
